How can you breathe in a corset?
Simple, my corsets a made to constrict the fleshy part of the torso at the waist only, not the ribcage or lungs, or the hip bone.  While they will provide nice shaping of the hips and chest they do not reduce these areas as they do the waist.

And is it painful? 
No, a properly fitted corset is not painful, so long as you give your body and your corset time to break each other in.  Actually, I find them quite comfortable.  They give you great posture and a shapely figure.

I've never worn a corset before, how much of a waist reduction should I order?
It is recommended for first time corset wearers to order a corset 3 to 5 inches smaller than their natural waist measurement.  While this may seem like a lot the fleshy part of the body gives quite easily, especially with persons with a natural waist over 32".  Customers with natural waists over 34" I usually recommend a 5" reduction.
